By submitting this form (whether by the artist or artist representative), I authorize the following: I hereby grant and authorize Sue Bonzell, Inc/Up N Country (hereinafter referred to as 'Company') the irrevocable and exclusive right to: Take, edit, alter, copy, exhibit, publish, distribute, and make use of any and all video recordings and materials depicting me, or that I have provided, for any lawful purpose, including but not limited to promotional, marketing, commercial, and editorial purposes. Utilize any and all video content, regardless of language, media, format, or market, now known or later discovered, in perpetuity. Continue to use the authorized content indefinitely, subject to mutual agreement, with the sole condition that the usage does not disparage my reputation or public image. Release me from the right to inspect or approve any finished product in which my likeness appears. Acknowledge that I have either received compensation for the use of my likeness or have willingly agreed to this release without compensation, waiving any entitlement to royalties or any other form of compensation, whether related to the use of the video in its current form or any derivative work created in the present or in perpetuity. Understand and accept that all materials, including video recordings and music provided by me, shall become the exclusive property of the Company and will not be returned. Additionally, I authorize the use of my music, which I have provided, on the 'Up N Country' show, in any format or medium, including but not limited to video, audio, radio, television, or any other format, wherever it is aired. Hereby release and hold harmless Sue Bonzell, Inc/Up N Country from all liability, claims, petitions, and causes of action, including those by my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate, arising from or related to the use of the authorized content. This release is legally binding and constitutes the entire agreement between myself and Sue Bonzell, Inc/Up N Country regarding the use of the authorized content.
